Esophageal cancer is relatively rare and is very deadly. In 2001, there were approximately 13,200 new cases of esophageal cancer diagnosed in the United States and nearly 13,000 deaths making esophageal cancer one of the most deadly of all cancers.
Cancers of the upper two thirds of the esophagus usually arise from squamous cells and are called squamous cells cancers. Cancers of the lower esophagus most often arise from columnar epithelium and are called adenocarcinomas. There has been a tremendous increase in the number of adenocarcinoma cases in the last two decades. Between 1/3 to 1/2 of all esophageal cancer diagnosis is due to adenocarcima. Barrett’s esophagus is a risk factor for adenocarcinoma.
Esophageal tumors usually lead to many symptoms including difficulty swallowing and pain. Small and localized tumors are treated surgically with the intent to be curative. Larger tumors usually are not operable and hence cannot be cured; their growth can still be delayed with chemotherapy, radiotherapy or a combination of the two. In some cases, chemotherapy and radiotherapy can render these larger tumors operable.
Outcomes of treatment for both squamous cell cancer and adenocarcinoma of the esophagus are very similar. The 5-year survival rate for all patients diagnosed with esophageal cancer is approximately 5%. This low survival rate is due to factors such as advanced age, the presence of local and distant spread of cancer at diagnosis, associated heart and lung disease and most patients have residual cancer remaining after primary treatment with surgical resection.
