Ovarian Cancer Subtype Changes the Treatment Strategy
Epithelial ovarian, fallopian-tube and primary peritoneal cancers share staging and many treatment pathways. High-grade serous carcinoma is the most common epithelial subtype and often begins in the fimbrial end of the fallopian tube. Low-grade serous, endometrioid, clear-cell and mucinous carcinomas have different molecular patterns, treatment sensitivity and clinical behavior. Germ-cell and sex-cord stromal tumors are biologically distinct and often occur in younger women.
A pathology label such as “ovarian cancer” is not enough for individualized care. We review histologic subtype and grade, surgical staging, sites of spread, treatment response and whether expert gynecologic pathology review is appropriate. A plan designed for high-grade serous cancer should not automatically be applied to low-grade serous or clear-cell disease.
Platinum Sensitivity and DNA Repair
Surgery, platinum-taxane chemotherapy, bevacizumab and biomarker-informed PARP maintenance may be considered according to stage, response, BRCA/HRD status and risk.
Hormonal and MAPK Biology
This usually slower-growing cancer is less chemotherapy-sensitive. Surgery, endocrine treatment and—in KRAS-mutated recurrence after prior systemic therapy—avutometinib plus defactinib may be relevant.

Expert pathology, stage and molecular features can alter the value of standard ovarian regimens, clinical trials and site-specific strategies.
Different Tumors, Different Priorities
Tumor markers, fertility goals, surgical approach and drug choices differ substantially from epithelial ovarian cancer.
Pathology, Surgery and Biomarkers Build the Road Map
Evaluation commonly includes pelvic and abdominal imaging, chest imaging when appropriate, CA-125 or other subtype-specific tumor markers, and review by a gynecologic oncologist. Tissue diagnosis is essential unless the clinical situation supports proceeding directly to surgery. When advanced disease is suspected, the team considers whether primary cytoreductive surgery or biopsy followed by neoadjuvant chemotherapy offers the safer and more effective sequence.
All women with epithelial ovarian cancer should discuss germline genetic testing, including BRCA1 and BRCA2, because an inherited result may guide treatment and inform relatives. Tumor testing can identify somatic BRCA variants, homologous-recombination deficiency, mismatch-repair deficiency or other actionable features. Folate-receptor-alpha testing can identify candidates for mirvetuximab soravtansine in specific platinum-resistant settings, while KRAS testing can matter in recurrent low-grade serous ovarian cancer.
Recurrence Is a New Decision Point, Not the End of the Plan
Time since platinum treatment, prior benefit, residual toxicity, disease location, symptoms and biomarker results all influence the next step. Options can include another platinum combination, non-platinum chemotherapy, bevacizumab, antibody-drug conjugates, endocrine or MAPK-directed therapy for selected low-grade serous disease, surgery in carefully chosen cases and clinical trials.
Surgery and Systemic Therapy Can Be Individualized Across the Journey
For many epithelial ovarian cancers, the goal of cytoreductive surgery is to remove all visible disease when that can be achieved safely. Surgery is often followed by carboplatin plus paclitaxel. Some women benefit from chemotherapy first and interval surgery after response. Bevacizumab may be added in selected advanced or recurrent settings, and maintenance treatment may include bevacizumab or a PARP inhibitor depending on disease risk, response, BRCA/HRD status, prior therapy and current regulatory guidance.
At recurrence, the treatment sequence becomes more individualized. Platinum-sensitive disease may respond again to platinum-based combinations. Platinum-resistant disease may be treated with single-agent chemotherapy, bevacizumab-containing regimens, folate-receptor-targeted mirvetuximab for eligible FRα-positive tumors or a clinical trial. Low-grade serous cancer can involve endocrine therapy and, for eligible KRAS-mutated recurrent disease after prior systemic therapy, the FDA granted accelerated approval to avutometinib plus defactinib in 2025.

The evidence for lifestyle interventions specific to ovarian cancer is still developing, but physical activity is associated with better health-related quality of life in available studies. Match Every Supplement to Surgery, Chemotherapy and Maintenance Treatment
Concentrated botanicals and supplements can affect platelets, bleeding, anesthesia, liver enzymes, blood pressure, glucose and drug metabolism. This matters around major abdominal surgery and during paclitaxel, carboplatin, bevacizumab, PARP inhibitors, mirvetuximab, endocrine therapy or MAPK-pathway treatment. St. John’s wort can lower exposure to some oral drugs, while grapefruit and other CYP-modifying products may raise or lower drug levels depending on the therapy.

Please include germline and tumor genetic testing, BRCA and HRD results, mismatch-repair or MSI testing, folate-receptor-alpha testing, KRAS results for low-grade serous disease, current blood counts and organ-function tests, and every prescription, nonprescription medicine, herb and supplement. Questions About Integrative Ovarian Cancer Care
Can Stage IV ovarian cancer still be treated?
Yes. Stage IV disease is serious, but treatment can reduce disease, relieve symptoms and sometimes control cancer for meaningful periods. The plan may include chemotherapy, surgery in selected situations, bevacizumab, maintenance therapy, biomarker-directed treatment and clinical trials.
Can natural therapies cure ovarian cancer?
No natural therapy has been proven to replace cytoreductive surgery, chemotherapy or an effective biomarker-directed treatment. Integrative care may support nutrition, strength, symptoms, recovery and quality of life while being honest about what is known and uncertain.
Should every woman with ovarian cancer have genetic testing?

What does platinum-sensitive or platinum-resistant mean?
These terms describe how the cancer behaved after platinum chemotherapy and help guide recurrence treatment. They are clinical categories rather than permanent identities; the exact interval, previous response, residual toxicity and current disease all matter.
